Description: A multidisciplinary group consisting of Quality Improvement Engineers, staff RN's, Nursing Leadership, and the Decedent Care Center Supervisor met to discuss methods to improve communication and the process when an unexpected death occurred in the Operating Room. We developed a workflow in Epic to guide staff on what to do in the event of a Patient death. The debrief development stemmed from a lack of communication and standardized workflow when unexpected deaths occur.
