Wound Ostomy Continent Nurse

Lianhong Li is a dedicated Certified Wound Ostomy Continence Nurse (CWOCN) at Thomas Jefferson University Hospital (TJUH) in Philadelphia, Pennsylvania. TJUH is an acute care facility and a Level 1 Trauma Center over 900 beds, as well as an academic institution with over 2,000 staff nurses.
In her role as a CWOCN at TJUH, Lianhong provides pressure injury prevention, ostomy care, and continuance care education to nurses, physicians, students, patients and their families. Her teaching methods varies from one-on-one shadow to formal presentations with CE accreditations. Lianhong enjoys both equally and embraces the opportunity to support health care staff in providing highest level of care to patients.
In addition to seeing patients daily and making appropriate wound care recommendations, the majority of her practice is dedicated to the prevention of hospital acquired pressure injuries. Being at the bedside as a clinician allows her not only to see patients and provide recommendations but more importantly, to interact with staff, observe their practices, and empower staff nurses.
Lianhong developed the skin champion committee and HAPI committee at her previous facility. She is also a member of our Hospital Acquired Pressure Injury (HAPI) Committee, a multidisciplinary committee tasked with reduction of HAPIs. Additionally, she has planned and coordinated a Global Pressure Injury Prevention Conference, an annual Conference at Jefferson that provides staff with opportunity to celebrate a worldwide pressure injury prevention day dedicated to a full day of CE accredited education as well as hands-on training.
