74 - Enhancing Safety and Efficiency in the Perioperative Suite

Description: In the dynamic and high-stakes environment of the perioperative suite, optimizing both patient safety and operational efficiency is a persistent challenge. This clinical improvement project aimed to address these issues through a multidisciplinary initiative that engaged perioperative nurses, surgical technologists, anesthesiologists, and surgeons in a collaborative effort to streamline workflows and enhance communication. Perioperative teams today face mounting pressures to deliver safe, efficient, and high-quality surgical care amid increasing patient volumes, staffing shortages, and evolving regulatory demands. Despite technological advancements, many operating rooms continue to experience inefficiencies that compromise both patient safety and staff well-being.

According to AORN, first case on-time starts occur only 64% of the time, and average turnover times often exceed 30 minutes, leading to significant downstream delays and increased operational costs. These inefficiencies not only affect scheduling and resource utilization but also contribute to staff fatigue and patient dissatisfaction.

Communication breakdowns remain a leading contributor to adverse events in the surgical setting. The Joint Commission's 2023 Sentinel Event Data Review reported that communication failures were a root cause in over 70% of sentinel events, which include wrong-site surgeries, retained surgical items, and delays in treatment. The National Patient Safety Goals continue to emphasize the importance of standardized communication tools such as time-outs and handoff protocols to mitigate these risks.

The team implemented evidence-based interventions including a standardized handoff tool, checklists, revised time-out utilizing and incorporating a TEAMSTEPS approach. Staff were engaged through interactive training sessions, staff meeting/huddle board, and leadership rounding. Post-implementation data demonstrated a 20% improvement in first case on-time starts and a 25% reduction in supply waste, turnover time, and surgical errors. Staff satisfaction with communication and workflow clarity also improved significantly.
