Description: The Case Cart Auditing Tool was developed to address recurring issues with improper surgical table breakdown and incomplete transfer of instrument trays to the Sterile Processing Department (SPD). These lapses not only disrupted SPD workflows but also posed potential safety concerns and delays in instrument reprocessing. The purpose of the tool is to ensure consistent adherence to table breakdown policy, ultimately supporting a more efficient and standardized instrument turnover process. The goal of this tool is to ensure that perioperative staff consistently follow established table breakdown protocols and adhere to proper instrument handling practices, supporting both patient safety and efficient SPD workflow.
The auditing tool consists of 14 structured questions that assess staff compliance in post-procedure tray preparation, with questions assessing adherence to Manufacturer Instructions for Use (MIFU), proper use of enzymatic spray, safe handling and disposal of sharps and single-use devices, and inspection for bioburden and damaged instruments. Observers complete audits in real time and provide immediate corrective education when necessary.
Since the implementation of the tool, there has been a marked improvement in the consistency and quality of trays sent to SPD. More trays are arriving fully broken down, properly organized, and compliant with reprocessing standards. On-the-spot coaching has increased staff accountability and reduced errors related to instrument damage and bio-burden on instrumentation. The auditing tool has proven to be an effective mechanism for improving surgical team performance, streamlining SPD processes, and enhancing overall patient safety.
